We are conveniently located at the Meadowbrook Mall in Boylston, Massachusetts. The name on our office is N. Thomas LaCava, M.D.

Driving directions

From I-190 north or south, take Exit 4.

At the intersection of Rt 12, take a left.

Continue past the gas station on your left.  The entrance to the Meadowbrook Mall is the next left.

We are located on the far left side of the mall. Park in any convenient location near our entrance.

From the South (Rhode Island area):
Take I-95 North or I-295 North, to Route 146 North.  Follow all the way to the Worcester, MA area.  Take I-290 East.  When on I-290 East, take exit 19 for I-190 North.  When on I-190 North, take exit 4, for Route 12. 

From the East:
Take I-495 South, to I-290 West.  From I-290 West, take exit 19 to I-190 North.  When on I-190 North, take exit 4, for Route 12.  

From the Boston area:
Take the Massachusetts Turnpike (I-90) West, to exit 10, I-290 East.  From I-290 East, take exit 19 for I-190 North.  When on I-190 North, take exit 4, for Route 12.  At the end of the exit ramp at the lights, take a left onto Route 12.   About 300 feet north, just past the first gas station on your left, take a left into the first shopping plaza, the Meadowbrook Plaza.  We are in the left corner of this plaza.

From the North:
Depending upon your location, take Route 2, to I-190 South.  From I-290 South, take exit 4, for Route 12.  At the end of the exit ramp at the lights, take a left onto Route 12.

From the West (Springfield area):
Take the Massachusetts Turnpike (I-90) East, to exit 10, I-290 East.  From I-290 East, take exit 19 for I-190 North.  When on I-190 North, take exit 4, for Route 12.
